New Patient Registration

The Practice is currently accepting new patients. The Practice accepts new patients who live within our practice area. This is the area bounded by Bushbury, Pendeford, Oxley, Fordhouses, Whitmore Reans, Coven and Low Hill.

As it often takes some time for records to be forwarded from your former practice all newly registered patients will be asked to complete a health questionnaire and are offered a consultation with the doctor. 

Patients do not need to show proof of address/ID/or immigration status to register

How to Register With Us

You can visit the practice to obtain the registration form, or alternatively you can download the form here, and bring it to the practice once completed.

Please bring the completed form along to reception with your medical card/NHS Number if you have it; having your NHS Number will speed up the process of getting your records from your previous practice.

Once you have been accepted as a patient your medical records will be transferred to us. You do not need to inform your existing practice that you are moving, although it may be courteous to do so.

  • If you have recently had a baby, it is preferred by us if you register the baby with us as soon as possible. This will save any unnecessary waiting if you attend the practice baby clinic which is often very busy. It is preferred by Dr Mittal to have both parent and child registered at this practice, as we like to build relationships with families, but this is your choice entirely.
  • When registered you may make an appointment as soon as you require.
  • If you require help or assistance with how we contact you or you contacting us, either with leaflets or information in any given format, please let us know when you register with us. We are happy to contact you via fax/email/text if it is what you prefer, or it is easier for you.

Non-urgent advice: Notice

Contractual obligations state that we have to let you know within 14 days whether your application to join the practice has been successful. You will be sent a letter and a practice leaflet. All applications are carefully considered even if prospective patients are from outside our catchment area.

We may wish to contact you, please let us know if you consent to this.

Recent changes to information technology at the Practice mean we may send you text alerts/reminders and messages from AccuRx. Changes to this service mean your text alert will only be sent if you have booked 1 week in advance.


Overseas Visitors

Those who visit the UK from overseas must pay for their care when they are in England.

In 2015 changes were made to how the NHS charges overseas visitors for healthcare so the NHS does not lose out on income for the services these patients have received.


Temporary Registration

If you’re currently staying with a friend or family member within our GP surgery area for more than 24 hours but less than 3 months, you can ask to register as a temporary resident. You can find more information on registering as a temporary resident on the NHS website.
